This Evidence to Action Brief was made possible through UNICEF’s partnership with University of Oxford and University of Cape Town under the 2gether 4 SRHR programme to help translate new evidence into improved programming and outcomes. This evidence to action brief is part of a series focusing on programming for adolescents and young people. It demonstrates that “transition” in HIV care for adolescents living with HIV in ESA relates more to decentralization than moving from paediatric to adult services. It offers suggestions on how to ensure better outcomes along the continuum of care.
